How to register a clinic in Tamil Nadu: a step-by-step guide

Opening a clinic involves two kinds of registration: setting up the business, and getting the healthcare-specific approvals. This guide walks through both, in order.

Step 1: Choose your business structure

Most single-doctor clinics start as a proprietorship. Multi-doctor clinics often choose a partnership, an LLP or a Private Limited Company, mainly for limited liability and a clean ownership split. If you plan to expand to several locations or bring in investors, a company is usually the better base.

Step 2: Register the business

Depending on the structure, this means incorporation or a partnership deed, a PAN for the entity, a current bank account and, where needed, GST. Many core healthcare services are GST-exempt, but related supplies are not, so check your position before registering. See our GST for doctors guide.

Step 3: Clinical establishment registration

Clinics, hospitals and diagnostic centres are regulated by state law, and the registration process, forms and standards differ between states and by size of facility. Step 4: Other approvals you may need

  • Trade licence from the local body for the premises.
  • Fire safety clearance, depending on building type and size.
  • Biomedical waste authorisation under the Bio-Medical Waste Management Rules, for facilities that generate such waste.
  • Pollution control board consent where applicable.

Step 5: Set up accounting, payroll and compliance
